Let's convert the elixirs from Draconomicon and Dragon Magic for D&D 3.5.

  

Blood Elixir (Black)

A blood elixir is a concoction brewed from the concentrated blood of a true dragon. This elixir gives the imbiber a +2 bonus to ST and the Infravision advantage (p. B60).

He can breathe fire once during the elixir’s duration, dealing 2d burning damage to a single target up to 10 yards away. Use Innate Attack (Breath) to hit. Don’t apply range penalties; treat this as a long melee weapon rather than a ranged attack.

Blood Elixir (Silver)

A blood elixir is a concoction brewed from the concentrated blood of a true dragon. This elixir gives the imbiber Charisma 2 (p. B41) and the ability to walk on clouds. Clouds are like solid ground beneath subject’s feet, allowing him to walk on them at its ground Move. If the subject gets knocked down or slips, he falls! The subject may attempt one DX roll per second of falling. If he succeeds, he stops in the clouds, unharmed, if there are still any clouds beneath him. Otherwise, he hits the ground for normal falling damage (see Falling, p. B431).

Dracolich Brew

Before drinking this potion, the would-be dracolich must complete a preparation ritual. This ritual takes 30 days. These do not have to be continuous or consecutive, but any day devoted toward the ritual cannot be spent adventuring, working on other projects, etc.; the subject has only enough free time to eat, sleep, bathe, and perhaps converse a bit.

To complete the transformation, the would-be dracolich must drink a dracolich brew. When a subject drinks a dracolich brew, he must roll vs. HT. If he succeeds, he dies, rising as a lich in 2d days. If he fails, he just dies. Luck, as well as the Bless and Wish spells, can influence this HT roll. This elixir does not work on non-dragons.
